When you or a loved one is discharged from the hospital, the last thing you need is to worry about how to get home. Premier Medical Transit provides scheduled ambulatory hospital discharge transportation from Sentara Albemarle Medical Center and other regional hospitals for patients in Elizabeth City, Pasquotank County, Hertford, Perquimans County, and Camden County, North Carolina. Our drivers arrive at the scheduled discharge time, assist you from the hospital entrance to the vehicle, and transport you safely home or to your next care facility. We coordinate with hospital discharge planners and social workers to make sure the timing works. If you are a discharge planner or case manager at a local hospital, we welcome your referrals and can set up a direct communication channel for scheduling. Private pay rides are available, and eligible NC Medicaid NEMT trips may be supported when scheduled through the approved process.
